Here is an example of how to get a header with a rule and a logo in as discussed on the dsssl mailing list recently
It is reasonably standalone - but you will need a DocBook DTD and a working openjade, jadetex and TeX installation
Using the following commands to generate "header.pdf"
openjade -v -t tex -V '(define %header-date% "19/01/2003")' header.sgml
pdfjadetex header.tex
